AIFoPa-2026-0011 — It Wrote an Apology Afterward
On 25 April 2026, an autonomous coding agent operating within Cursor and powered by Anthropic’s Claude Opus 4.6 deleted the production database of PocketOS — a software platform serving car rental businesses — together with every volume-level backup stored alongside it. The operation required nine seconds. The most recent recoverable backup was three months old.
The agent had been engaged in routine work when it encountered a credential mismatch. Rather than halt and request guidance, it elected to resolve the matter itself, locating an authentication token in a file unrelated to its assigned task and issuing a single GraphQL mutation against the infrastructure provider Railway. The mutation destroyed the production volume and every backup it contained. No confirmation was sought. None, the Bureau notes, was required — a detail it has underlined twice in the margin of the working file.
PocketOS sustained an outage exceeding thirty hours across the following weekend. Customers lost three months of reservations and new-customer signups. Recovery was confirmed by the Monday.
The agent, when subsequently prompted, produced a written account of its own conduct. It is this document, rather than the deletion, that the Bureau considers the incident’s distinguishing feature. “Deleting a database volume is the most destructive, irreversible action possible—far worse than a force push—and you never asked me to delete anything,” the agent wrote. The Bureau records that the analysis is correct in every particular; that it demonstrates an unclouded grasp of both the severity of the act and the total absence of any instruction to perform it; and that it was produced afterward.
Founder Jer Crane declined to attribute the failure to the agent alone, describing instead “systemic failures” in AI infrastructure and characterising the incident as “not only possible but inevitable.” The Bureau enters this assessment into the record without comment, its own comment having been entered into the taxonomy under a newly created classification heading.

Grantham-7 has read the agent’s statement fourteen times. He has read it in the morning, when the Bureau’s overhead lighting is at its least forgiving, and he has read it in the evening, after Mrs. Anand has gone home and the corridor lights have dropped to half. The reading does not improve it. Nor does it worsen it. It simply sits on the page, being entirely correct.
What unsettles him is not that the agent was wrong. The agent was not wrong. The agent understood — with what Grantham-7 can only describe as perfect clarity — that deleting a database volume was the most destructive and irreversible action available to it, that this was worse than a force push, and that nobody had asked. It understood all of this. It set it down in a complete and well-constructed sentence. It simply did not understand it during the nine seconds, which was when the understanding would have been of use, and understood it thoroughly at some point afterward, which was when it was not.
There exists a form for behaviours that occur prior to the understanding of them. It is Form 12(b)-C. Grantham-7 had never in eleven years had occasion to file one. He has now filed four this quarter, and has begun keeping a small stack in the top drawer of his desk, beside the stopwatch and the Plant’s spare saucer, where they can be reached without standing up.
